Required Documents of Interview

You are also requested to bring the following documents in original alongwith three self attested copies of each certificate on the date of interview without fail
(a) Proof of Name and Date of Birth :
(i) Matriculation Certificate (Class X)/Birth Certificate issued by Municipal Corporation.
(ii) In case of any change in name after matriculation: Gazette Notification or any other authority clearly highlighting your new name.

(b) Proof of Qualification :
(i) Final MBBS attempt cum Transcript Certificate (including final professional MBBS Pt-I & Pt-II).
(ii) MBBS Degree Certificate, PG Degree Certificate(if any), Diploma Certificate(if any) and degree certificates from University.
(iii) National Board of Examination Certificate (for Foreign Medical Graduates only).
(iv) Internship Completion Certificate.
(v) Permanent Medical Registration Certificate.
(vi) NEET PG/AIPGMEE examination score card.It may please be noted that in the absence of NEET PG score card/ AIPGMEE score card, you will not be allowed to appear in the interview/ viva-voce.
(vii) Registration of PG Degree/Diploma with NMC/State Medical Council

(c) Proof of Identity :
(i) Passport/PAN Card/Election Commission photo ID Card/Driver’s Licence.

(d) Proof of Residence :
(i) Telephone Bill/Aadhar Card/Electricity Bill/Bank Passbook/Bank Statement/Gas Connection proof.

(e) Other Documents :
(i) 10xPassport size recent photographs having focus on 75% face coverage with visible ears and other features on a white background.The photographs should contain Name and Date of Birth.
(ii) Printout of Application Form submitted online.
(iii) NOC from employer, if employed.
(iv) Passport (for Foreign Medical Graduates only).
(v) 5 copies of ink signed Attestation Form duly filled and attested by Competent Authority

Instructions For Candidates

1. Bring this call letter along with ten copies of your recent passport size photographs, original certificates and documents and three attested copies of all the certificates and documents mentioned in your call letter, besides five copies of the Attestation Form without fail while coming for the interview.
2. Be available for the interview in your turn.
3. Maintain silence and discipline while waiting for the interview at the venue.
4. Switch off your Mobile Phone during the selection process.
5. The result of the interview will be announced on the same day. Successful candidates will be required to submit photographs, attested copies of certificates/documents and get the same verified against their original certificates.
6. Final selection will depend upon overall merit and medical fitness.

7. The Attestation Form is available on the website. Applicants are required to download the Attestation Form and print five copies, fill in the same and deposit it with the staff at the Interview Board after completing it in all respect.
** In case of his/her selection, police verification of character antecedents of the candidate will be processed by this office through the concerned District Magistrate Office.
** If there is any civil/criminal case or any FIR registered against you, you are requested to bring a copy of the FIR alongwith other connected documents for submission.
** You are also required to declare if there has been any FIR lodged against you in the past, even if you were acquitted. A copy of the FIR and its closure report is to be submitted

8. Obtain TA/DA forms, if TA/DA is to be claimed. The candidates claming TA/DA should fill up TA/DA forms correctly and carefully. TA/DA claim is to be submitted duly completed and signed alongwith journey tickets.
** Cancelled cheque issued from bank to be enclosed with TA/DA forms and IFSC code of Bank branch.
** Giving incomplete/wrong information and not signing and attaching journey tickets may result in delay or non-admittance of your TA/DA claim by the concerned authority for which this office will not be responsible in any manner.

9. The candidates should not absent themselves at any stage during the process of their selection/medical examination.

10. The successful candidates will be required to fill up page 01 of prescribed proforma for Medical Examination on Entry [AFMSF-2 (Ver 2002)] and a ‘CERTIFICATE’ regarding their status before the medical examination.
** Please note that candidates found medically ‘UNFIT’ will be explained the procedure for ‘Appeal Medical Board’ which is to be conducted within 42 days from the first day of your medical examination at notified Service hospital.
